Community Nurse – End of Life Care Coordinator

  • Tell us about your role. What do you get up to on a typical day/week?

    My role involves managing a caseload and working collaboratively with the Fast Track Team, Hospice at Home and Marie Curie services. We take in referrals and allocate visits according to priority. I also conduct visits for assessment of patient needs, providing personal care and palliative support to patients and families.

  • What’s the best thing about working in the community? What made you choose this kind of work?

    Working in the community gives you a little more time with patients and families, which provides an opportunity to build rapport and get to understand patients’ situations in more detail. I can follow through with patients and see the implementation of the advice given and the services they receive. It gives me instant job satisfaction, which is very rare.

  • If you have worked in a previous environment, e.g. acute, how does this compare to working in community health?

    I worked in a hospital for nine years prior to joining BCH. In a hospital environment, there is a lot of pressure and conveyer belt type activity and a lack of opportunity to get to know patients. One of the things that attracted me to community health was the ability to spend more time with patients. There is also more opportunity and time to signpost patients to other services.

  • Describe the working culture here?

    There is a great team spirit. People work well with each other to get the job done. Teams are very proud of the services they provide and always strive to do their best.

  • What do you think makes someone successful here?

    Having a strong desire to learn new skills and be willing to work as part of a team in order to help each other to do the best job.

  • Tell us about the learning and development and career progression opportunities?

    When I worked in a hospital environment it took me many years to do something as simple as a mentorship course (FLAP). In community health, there is a lot more learning and development study days you can book. There is more opportunity to gain more skills, gain competence as a lone worker and develop individually. I love the variety of career pathways, competencies and specialist skills available to choose from.

    I am part of a program called Stepping Up, which is a joint venture with various employers (including BCH) and Bristol City Council towards creating a One City Plan. I feel proud to say that BCH encourages and embraces diversity and provides BAME employees the opportunity to air our views and impact equality and diversity. The Stepping Up programme has allowed me to really stretch myself and I have learnt a great deal on global trends and how to be a great leader, not just for BCH but for the city of Bristol and the South West region. I have had access to mentorship and coaching and looking at issues globally, which helps me to see the bigger picture and challenges me to take higher roles. The Stepping Up programme also provided ILM Level five training for me.

  • What’s the most rewarding thing about your role?

    We work in a highly emotional environment. My role is to help people reach their final destination – dying in their preferred place. It gives me the opportunity to help patients and their families to fulfil their final wishes and help them through their journey, providing support through death of a loved one and bereavement.

  • What do you think patients would say about the service you provide?

    The feedback we receive from patients is consistently positive, they say how much we have helped them by treating them with dignity and respect and helping families to cope. We help the families of patients to maintain their role as a family member rather than a carer.

    I am also a member of the Staff Council which gives me the chance to take part in shaping up and understanding where BCH is going. I would encourage everyone to contact the Staff Council members in your areas and get your views, issues and any suggestions across. Your input is very important to BCH.
