The Community Discharge Co-ordination Centre manages all rehabilitation referrals from the acute hospitals into Community Rehabilitation Beds. We are looking for an enthusiastic health care professional to join our team.

As a CDCC case-manager you will be part of a multi-displinary team responsible for completing assessments for community rehabilitation (pathway 2) beds and co-ordinating discharges. You will utilise your assessment, problem-solving, clinical reasoning and communication skills on a daily basis to work with patients, their families and the multi-displinary team to develop discharge plans and identify the most appropriate place for the patient to continue their rehab journey.

The Case-manager team in-reaches into Southmead Hospital, Bristol Royal Infirmary and South Bristol Community Hospital. You will need to be flexible to work across locations depending on service demands.

You will need to have a good understanding of rehabilitation pathways, discharge planning and mental capacity.

In return, we will give you the opportunity to develop skills in leadership and multidisciplinary working; provide you with regular training and supervision and enable you to become involved in future service developments.

The Service operates Monday – Saturday, between 08:00 – 18:00 and offer flexible working patterns.
