Band 7 Clinical Team Lead (Learning Disability Nurse)
1 WTE 37.5 hours per week
An exciting opportunity has arisen for an experienced Band 7 Learning Disability Nurse to join our Community Learning Disability Team in Bristol Community Health.
You will work jointly with your Clinical Lead colleague to manage the ‘Accessing Health and Wellbeing Team’ within the CLDT. This team is focused on ensuring a multi-disciplinary approach to supporting service users to access mainstream services where possible, as well as supporting primary care, health promotion work and those in hard to reach groups.
By working alongside mainstream health services, you will support them to make reasonable adjustments and increase their knowledge of how to successfully work with people who have a learning disability.
You will be a key member of the multi-disciplinary team, contributing to a range of interventions with other healthcare professionals; attending MDT meetings, setting person centred goals and working in partnership to achieve improved health outcomes.
As well as providing clinical and managerial leadership for the team, you will have the opportunity to lead on projects in partnership with other agencies in order to meet the key findings from the LeDeR programme.
