We are now looking for an experienced clinician – whether this is a nurse, physiotherapist, paramedic or occupational therapist– to use their experience to help manage a complex caseload in our Single Point of Access (SPA) Team. Could this be you?

What you’ll do

The Single Point of Access (SPA) completes the office based triage of healthcare referrals and manages capacity for our Community Nursing and Urgent Care Services (Rapid Response, COPD and Urgent Therapy). Referrals come into the service via phone, email and by electronic systems.

In this role, you’ll be working as a case manager, ensuring all patients and clinicians that access the service receive the support they require. Utilising your clinical knowledge, problem solving skills, leadership skills and experience, your responsibilities will include:

  • Coordination and Management of capacity across the above services
  • Triage of referrals for Community Nursing and Urgent Care services
  • Maintaining high levels of clinical knowledge and decision making
  • Assessing and planning nursing treatments for patients in a variety of community based settings, preventing unnecessary hospital admissions and facilitating early discharge.
  • Identify when additional clinical input is required, and refer on to the appropriate specialist if deemed necessary.

Where you’ll work

You’ll be based at the Withywood Centre in south Bristol. The Centre is a key hub for healthcare provision within the area, providing as well as the SPA service health clinics for patients, community and social activities and courses, a community café, and more. The site also offers ample staff parking.

What we’d like from you

You’ll be an experienced clinician with a registered qualification (e.g. NMC, HcPC, GPhC, etc.), with experience within health or social care within acute/community environments. This will ideally include having worked with individuals with long-term conditions and older people. You’ll also be comfortable working in a multidisciplinary team, containing Nurses, Physiotherapists, Occupational Therapists and admin staff. On top of this you’ll have an understanding of the struggle for delivering health services, such as patient pathways and triage. Meanwhile as a person you’ll be a great team player with strong communication skills towards patients and fellow staff alike.
