We are currently looking to recruit enthusiastic and motivated Band 4 Community Practitioner: Nurse Associate or Assistant Practitioner to join our Community Nursing Teams in Horfield and Shirehampton.
As a Community Practitioner you will be expected to provide support to our patients in a wide range of clinical community environments, along with supporting clinicians with their patient workloads. You will be responsible for the holistic, comprehensive assessment of more complex community patients, delivering clinical care delegated by a registered professional and contributing to the evaluation of person centre care plans.
The role requires excellent communication skills, IT skills, and the ability to be adaptable, reliable and highly organised. Experience of care in a community setting would be an advantage but not essential as a comprehensive induction programme is provided.
In order to be considered for this role you need to either have a foundation degree / NVQ Level 4 in Health and Social Care or Registered Nurse Associate qualification. You must also be able to travel to various locations to visit patients in their homes; therefore access to a motorised vehicle is essential.
