This is an exciting new role to support our expanding Home First Team. We are looking for an enthusiastic and motivated individual to join us.
As the Home First Co-ordinator you will be responsible for planning, co-ordinating and scheduling visits with support from clinicians and an administration team. This will include daily liaisons with patients, families and health professionals to establish the best discharge plan for the patient from hospital.
A core aspect of this role will be developing relationship with voluntary services and social care partners to pro-actively support the assessment, care and rehabilitation plans.
This role would be suitable for someone with experience of working in a health or social care setting who is looking to progress into a leadership role. You will operationally manage therapy assistants within the team and plan their daily workload with support from a duty clinician.
You will predominately be based within the Community Discharge Co-ordination Centre, located in the city centre working as part of a team to manage referrals into the service, however you will have the opportunity to support the team on visits and maintain or develop your clinical skills within the community.
You will need excellent communication and IT skills.
You will be supported to develop your leadership and managerial skills and receive regular supervision. As well as having the opportunity to shape this new role and participate in service developments.
The Service operates Monday – Saturday, between 08:00 – 18:00.
