Our dedicated Community Therapy Teams are made up of Occupational Therapists, Physiotherapists, and Rehabilitation Support Workers. We offer the opportunity to work closely with a number of specialists including speech therapy, mental health and other BCH specialist teams such as neuro, parkinsons and respiratory. Partnership working is key to what we do working closely with our council colleagues, voluntary sector, general practice and other private organisations.
Your role within our committed and motivated teams will be to complete holistic assessments and treatment plans, to support patients to meet their goals, manage their own conditions and live as independently as possible within their own homes.
We are always looking of ways to ensure we are developing and engaging our staff and as result we can offer a wide variety of job focused activities. This includes team building days and support for furthering clinical and leadership skills and knowledge on relevant CPD, internal and external courses. We welcome both individuals new to leadership and management and those with experience who wish to be stretched and challenged in a new setting. We recognise and encourage new ideas and nurture those with specialist interests.
As well as providing rehabilitation within the community therapy teams, you will also have the opportunity to support the Homefirst team in same day hospital discharge assessments in patient’s own homes to ensure safe and effective discharge. This includes functional and medical assessment and setting care plans for reablement support workers.
As a part of the Community Therapy Team, you will have regular line manager 1:1 and clinical supervision, the opportunity for peer supervision, as well as supporting and developing Band 5 therapists, Rehabiltation Support Workers and students. BCH also actively encourages and involves staff in service development of both the team and overall city wide therapy services.
We pride ourselves on supportive team work, focusing on patient centred goals using shared decision making techniques, providing community rehabilitation to those with both acute and chronic conditions such as; stroke, amputations, progressive neurological conditions, orthopaedic trauma, frailty and falls.
