We need an experienced Nurse to work as a Band 6 Case Manager.
The Single Point of Access (SPA) manages all referrals and capacity for Community Nursing and Urgent Care Services (Rapid Response, COPD and Urgent Therapy). Referrals come into the service via phone, email and by electronic referral.
The successful candidate will be involved in the office based telephone triage, assessment of needs and co-ordination of referrals into the community teams.
You will ideally have community experience, having worked with individuals with long-term conditions, and be comfortable working in a multidisciplinary team, containing Nurses, Physiotherapists, Occupational Therapists and admin staff.
Your responsibilities will include:
- Triage of referrals for Community Nursing and Urgent Care
- Management of capacity across the above services
- Working as a shift leader for the service
- Maintaining high levels of clinical knowledge and decision making

- E rostering up to 12 weeks in advance
