Would you like to be part of an exciting, dynamic team? The Community Discharge Coordination Centre manages all referrals to community therapy services both from hospital and the community. The service is currently expanding to support the HomeFirst Service as it is a key competent of the ongoing and future success of this development.
We are looking for someone who is dynamic and able to adapt and flex with changes to the service. All staff are encouraged to take an active part in service development and help influence change. You would be joining a supportive, proactive and highly valued team.
Your role will be to provide admin support and a central function to support our clinical therapy teams within the hospitals, HomeFirst and community team. The main purpose or the team is to ensure smooth and timely transfer of care, to guide patients through their rehab journey. Many tasks will include; processing new referrals, updating spreadsheets, managing waiting lists, completing reports relating to capacity, information gathering and liaising with health, social care professions, patients and their families.
You will need to have excellent communication skills, experience of working as part of a team and have a high level of proficiency in Excel and word processing.
