Since 2011, we’ve been working flexibly and collaboratively to touch the lives of thousands of people, enabling them to stay in their homes where they are most comfortable and saving public money along the way.
In that time, we’ve supported over 100,000 people and delivered 1.2 million moments of outstanding care. We’ve prevented over 35,000 hospital stays and saved £20 million. And we’ve done it all with quality front of mind, achieving 93% of national and local quality objectives and 96.5% high quality care. Our approach is patient-centred and 97% of our patients say they would recommend us to friends and family members.
We’ve reached out to our communities increasing patient feedback, publishing community newspapers and providing interpreting services to over 75,000 people.
We’ll continue to work with patients, partners and the community to improve our services even more. And we’ll continue to invest in our people too, listening at engagement sessions and recognising success with awards.
